Canva + Google Form Template for Real Estate Agents
Open House Feedback Form Template
Find out what visitors actually thought — while the home is still fresh.
Most open house feedback lives in your memory by Monday: a few half-remembered comments, a couple of names, nothing you can act on. This template gives visitors a clean way to tell you what they thought from their phone — editable Canva QR signs plus a ready-made Google Form — so you end the day with real reactions instead of a blur.
Edit the sign, point the QR at the included form, and set it out after the tour. The responses come back organized and ready to read.

What this template helps you do
A casual “what did you think?” at the door gets you a polite “nice place.” A short form a few steps later gets you the truth: what they liked, what gave them pause, and whether they want to hear from you.
Use it to:
- collect honest reactions while the showing is fresh, not days later
- capture what buyers liked, what concerns came up, and how interested they are
- find out who wants follow-up, similar listings, or nothing
- give your seller a real read on the open house instead of “it went well”
- start follow-up from organized responses instead of memory
Who this is for
Best for real estate agents who want a clear read on an open house — both for their own follow-up and for the seller update afterward.
A good fit if you:
- want feedback you can actually use, not hallway small talk
- prefer visitors share from their phone over a paper comment card
- are comfortable editing in Canva and copying a Google Form
- want seller-facing feedback to summarize after the event
Not ideal if you want a CRM, automated outreach, or a done-for-you lead system. This collects the feedback cleanly; what you do with it is your call.
What’s included
The display and the form destination, both ready to customize:
- 3 editable Canva QR feedback signs — “Tell Us What You Thought / Scan to Share Feedback” layouts in three branding options: agent headshot, headshot plus brokerage logo, or logo-only
- 1 ready-made Google Form for open house feedback, with the questions already built: name, email, phone, working-with-an-agent, overall impression, what they liked most, concerns or objections, interest level, follow-up preference, and questions or comments
- 2 editable Canva Google Form header templates to brand the form itself
- QR code replacement instructions
- Quick-start PDF setup guide
Clean black-and-white layouts that sit well next to most real estate branding.
How it works
1. Customize the sign in Canva.
Add your name, photo, logo, and contact details to one of the three layouts. Works with a free Canva account.
2. Copy the Google Form.
Make your own copy. The feedback questions are already built — edit only what you want.
3. Point the QR at your form.
Copy your form link and update the QR on the sign, using the included instructions.
4. Set it out after the tour.
Place the sign by the exit or on the table so visitors scan on their way out.
5. Read the responses.
Feedback comes back in Google Forms — organized and ready to act on or summarize for your seller.
Connect it to your sign-in form
If you also run a QR sign-in at the door, the two forms chain together. Paste your feedback-form link into your sign-in form’s confirmation message, so after a visitor signs in they see something like: “Thanks for signing in — after your tour, you can share your feedback here.”
That creates one clean flow: sign in first → tour the home → share feedback after. The feedback form also works completely on its own, with no sign-in form required.
Works well with
The feedback form runs on its own, and it’s a natural part of a fuller open house workflow:
- the open house QR sign-in to capture visitors at the door (and chain to this form afterward)
- the open house lead follow-up tracker to sort responses into follow-up categories with timing and messages
Compatibility and setup notes
This uses Canva and Google Forms — a free Canva account and a free Google account are all you need, and it’s built with Canva’s free elements (no Pro subscription required). Canva occasionally changes which elements are free; if you ever hit a Pro-flagged item, it’s quick to swap for a free alternative or remove it. You’ll add and scan-test your own QR code before the event. It doesn’t send anything automatically; it collects feedback for you to review and act on. Digital template; nothing physical ships.
Common questions
Do visitors need an app?
No — they scan with their phone camera and the form opens in the browser.
Do I need Canva Pro?
No. It’s built with Canva’s free elements and edits on a free account.
Is the Google Form ready, or do I build it?
Ready-made. The feedback questions are already built — copy it and edit only if you want to.
Can I connect it to my sign-in form?
Yes — paste the feedback link into your sign-in form’s confirmation message so visitors get it right after signing in.
Can I reuse it for other listings?
Yes. Copy it and update the details per property.
Can I change the questions?
Yes — the form is fully editable in your own Google account.
Ready to hear what visitors really thought?
Set out a feedback sign, point it at the ready-made form, and end the open house with reactions you can use.
